- Legal name as it appears on your photo government issued drivers license
- Contact phone number (cell number)
- Frequent flyer number (if applicable)
- Date of birth

All tickets are non-refundable. Changes allowed for a fee and subject to any fare increases.
**If you wish to purchase flights with companion vouchers, frequent traveler point etc, you will be required to make your own reservation. Booking made via one of these channels will not be reimbursed by the Western Canadian Cystic Fibrosis Conference.
